VISA FOR CHAD

  1. ACTUAL PASSPORT

    You must send your actual passport. PASSPORT MUST BE SIGNED. The visa will be stamped on a page in your passport. Your passport must have at least 6 months of validity left beyond your intended stay, and must have at least 2 blank visa pages side by side. Amendment pages (pages 22, 23 and 24) do not count.

  2. THREE VISA APPLICATION FORMS FOR CHAD

  4. THREE PHOTOGRAPHS

    Each applicant should submit three identical color photographs. Photographs should be 2" x 2" in size, taken against a plain white background. Snapshots or computer pictures will not be accepted.

  5. VACCINATION FOR YELLOW FEVER

    International certificate of vaccination for yellow fever is required.

  6. FLIGHT ITINERARY

    Flight itinerary or copy of round trip tickets is required.

  7. PROOF OF U.S. STATUS (For non-U.S. citizen only)

    Non-U.S. citizen should submit proof of U.S. status (e.g. photocopy of U.S. permanent resident card or photocopy of U.S. visa.)

  8. BUSINESS LETTER (For business only)

    Provide a business letter on company letterhead giving details of the nature of visit, including the dates of stay, hotel information and addresses, organization to be visited and a financial guarantee from the company, signed by a senior manager.

  9. EMBASSY FEE

    $100.00 per visa - 1 month single entry
    $150.00 per visa - 3 month multiple entry
    $200.00 per visa - 6 month multiple entry

    Regular processing takes approximately 5 business days. 3 day rush available for an additional $25.00 per visa to the embassy.

  10. SERVICE FEE

    $39.00 per visa - 5 business day processing
    $75.00 per visa - 3 day rush

  11. RETURN FEDEX EXPRESS

    $23.00 per address (up to 3 passports)
    $28.00 per address (4 to 6 passports)
    $35.00 per address (Alaska, Hawaii and Puerto Rico or Saturday Delivery)
