Your valid U.S. passport. You must have at least 3 months of validity left on the passport beyond your intended stay in Azerbaijan. You must have at least one blank visa page. Amendment pages do not count.

One Visa Application Form for Azerbaijan, completed in full, giving particular attention to your entry and exit dates into and out of Azerbaijan. If the visa application is not completely filled out, signed and dated, we will not be able to submit your documents to the embassy, which will result in significant delays in the processing time.

Letter of Invitation from company or organization in Azerbaijan, authorized by the Azerbaijan Ministry of Foreign Affairs.

One Business Letter on company letterhead giving details of the nature of visit, including the dates of stay, hotel information and addresses, organization to be visited and a financial guarantee from the company, signed by a senior manager.
