Hong Kong Identity Card in ORIGINAL. If it is not available, fill out and submit a
Personal Information Sheet and Authorization Form.Two recent 2" x 2" full-faced color photographs with a light, plain background and taken less than 6 months ago. The face must be square to the camera with the mouth closed (teeth cannot be shown). Do not smile or grin in the photo as this will distort the normal facial features. One of the photographs must be countersigned by the person who has also countersigned the application. Applicant's relatives and notary public will not be accepted as countersignatory.

If you have changed your name since the issuance of the last passport, provide the original court order, marriage certificate, or divorce decree.

NOTE:
If you are renewing a red BN(O) passport that has expired for more than ten years, or renewing an old style blue BN(O) passport, you must submit your original birth certificate, a photocopy of state issued ID and a letter from the countersignatory on business letterhead certifying your identity.

If your passport has been lost or stolen or is unavailable you must provide the address at which you were living when the passport was first issued, the next of kin details included on your previous passport application form and the name of the person who countersigned the original application. For additional identification purposes please send your US passport or an enlarged clear copy (front and back) of your US Alien Registration Card and driving license (if held).
