- Were issued your most recent U.S. passport less than 8 years ago; AND
- Are using the same name on your most recent passport; AND
- Can show a valid reason for needing a second limited validity passport to The U.S. State Department.
REQUIREMENTS:
Your actual passport with at least two years of validity remaining. (Passport must be signed.)
Statement Requesting a Second Limited Passport, complete where applicable and signed by applicant.Letter from applicant, addressed to the U.S. Department of State, explaining the need for a second limited passport, signed and dated by the applicant. The letter may reiterate the reason stated on #3.
Letter from applicant's employer on company letterhead, addressed to the U.S. Department of State, explaining the need for a second limited passport, signed and dated by applicant's supervisor. The letter may reiterate the reason stated on #3.
- Two Letters of Authorization. These are form letters authorizing our company to process your passport request.
Two identical color photographs, 2" x 2" in size, with a plain white background and taken less than six months ago
Copy of travel itinerary from airline or travel agency showing conflicting flight schedules.

Government Fee:- Second Limited Passport Book: $135.00
- Passport Card (applying along with Second Limited Passport Book at the same time): $20.00

- $85.00 - 10 business day processing
- $125.00 - 5 business day processing
- $175.00 - 24 hour processing*

Passport Processing Order Form with credit card information. Application must be sent via FedEx First Overnight or
UPS Early AM Delivery, and must be marked "Signature Released" on package. Application that is not received at our office by 8:00am on the date reserved is subject to a charge of the full service fee. - Passport Card

Return FedEx Express:- $23.00 per address (up to three passports)
- $28.00 per address (four to six passports)
- $35.00 per address (Alaska, Hawaii and Puerto Rico, or Saturday Delivery)
