DS-11 EXECUTION INSTRUCTIONS

You may use Application for U.S. Passport (DS-11) if you:

  • Are applying for a U.S. passport for the first time;
  • Have a U.S. passport that was issued MORE than 15 years ago;
  • Have a U.S. passport, but were under the age of 16 when it was issued;
  • Have had a U.S. passport, but was lost or stolen (Also use Form DS-64); OR
  • Have a U.S. passport, but is now damaged or mutilated (Also use Form DS-64).

In order for Passports Plus to submit any of the above applications, you must go to an Acceptance Agent and have your documents witnessed, executed and sealed. You will need to first prove you are a U.S. citizen by providing one of the following:

  • A previous U.S. passport; OR
  • An original birth certificate with a raised seal, or a certified copy of your birth certificate. The birth certificate must include your first, middle and last names, date and place of birth, date the birth record was filed, and the seal or other certification of the official custodian of such records.

You must establish your identity to the acceptance agent. You may submit items such as the following containing your signature and physical description or photograph that is a good likeness of you:

  • A previous U.S. passport;
  • Certificate of Naturalization;
  • Certificate of Citizenship;
  • A driver's license (cannot be a temporary license or learner's permit);
  • Government (Federal, State or Municipal) employee identification card or pass.

The acceptance agent will then look at the required documents. After this, the agent will ask you to sign the DS-11 application form. The agent will ask you for a $25.00 execution fee, so you will need to bring 2 checks with you. The agent will then place all of the required documents into an envelope and seal them. Once the envelope has been sealed by the agent, it can be opened ONLY by a U.S. Passport Agent. If there is evidence that the envelope has been opened, the application will be rejected and you will need to start the process over again. Also, the envelope must be presented to the U.S. State Department within five business days of the envelope being sealed. Make sure that you DO NOT have our Passport Processing Order Form sealed in the envelope. This form must be paper clipped to the outside of the sealed envelope so that we can see the name, date of birth, and the date of departure for the applicant.
